Focus Physiotherapy and Ergonomics (Focus) consider your privacy to be of the utmost importance and we are committed to ensuring the confidentiality of your personal information. Our systems and procedures are designed to protect your personal information from unauthorised access or disclosure.

What information do we Collect?

We do not collect any personal information via our website.

If you require a consultation or treatment we will collect and store personal information only with your written consent. 

This information may include but is not limited to:

  • Name
  • Address
  • Date of Birth
  • Gender
  • Telephone number
  • Email address
  • Some medical history
  • Any treatments you have received
  • Place of work

What do you we use this information for?

The information is gathered for the purposes of providing you with a consultation and/or treatment and to communicate with you about your consultation or treatment. We do not use your information for marketing purposes.

How do we keep your information safe?

Data we hold on you is stored both electronically and we also maintain some paper documentation. The electronic data is secured by high level encryption. Some of the safeguards we use are firewalls and data encryption, physical access controls to our data centres, and information access authorization controls. Where information is sent by email this is also securely encrypted. Physical paper documentation is kept in secure locked cabinets in secure alarmed premises.

Who else has access to my information?

Only Focus staff who need to see your information for legitimate working purposes have access to your personal information. Your information will only be shared with your explicit consent and will only include necessary information. This may include, but is not limited to, your Doctor, your employer, or another health professional. We may also share your information as part of any legal obligation to do so.

Can I see the information you hold about me?

If you would like to see what information we hold you can ask us by submitting a ‘Subject Access Request’ (SAR). You can do this verbally or in writing. We may ask for proof of identity before providing sensitive information. We will reply to any SAR within 30 days.

What if I think the information is incorrect?

If you believe any personal information we hold is not correct you can submit a written request to have the information updated.

What if I don’t want you to hold information on me any longer?

You have the right to request that any information we hold be deleted from our records. We will confirm that this has been done. We maybe legally obliged to keep some information in a similar way to your doctor. Where we hold physical paper documentation and the legal requirement to maintain this documentation has passed it is destroyed and a destruction certificate obtained as proof.
